When teachers see a situation with a student that they would want to refer to an outside professional such as a special educator, psychologist or behaviorist, it is sometimes hard to objectively describe the situation for their benefit. Parents sometimes might have a different point of view of the situation. This form helps teachers to present an objective snapshot of the student’s situation. It provides the specialist with a well-defined and objective starting point.
The forms are mostly created in checklist mode in order to minimize the teacher’s work. There is minimal narration involved.
